Doctors led by Daniel Kripke of the Scripps Clinic Viterbi Family Sleep Center in La Jolla, California, looked at the medical records of more than 10,500 adults living in Pennsylvania who were taking prescribed sleeping aids. These were compared against more than 23,600 counterparts, matched for age, health and background, who did not take these drugs.

The study ranged over two and a half years, and looked at widely-prescribed sleeping pills, including benzodiazepines, non-benzodiazepines, barbiturates and sedatives. The overall number of deaths that occurred during this period was small in both groups, being less than a thousand in total.

But there was a striking difference in mortality, the researchers found. Those who took between 18 and 132 doses of the pills per year were 4.6 times likelier to die than the “control” group. Even those who took less than 18 annual doses were more than 3.5 times likelier to die. “Rough order-of-magnitude estimates… suggest that in 2010, hypnotics (sleeping pills) may have been associated with 320,000 to 507,000 excess deaths in the USA alone,” says the study.

Details of how individuals died were not disclosed, and the authors stress that they have found a statistical link but not a cause. But they sound the alarm, given the vast number of people who take these drugs. “We estimate that approximately six to 10 percent of US adults used these drugs in 2010 and the percentages may be higher in parts of Europe,” they write.

The average age of the people in the study was 54. The researchers say they took into account factors that could skew the comparison between the two groups, such as whether an individual smoked or had a pre-existing health condition.

However, they were unable to take depression, anxiety and other emotional factors into account, as these diagnoses are kept secret under Pennsylvania law. Previous research into sleeping pills has found a link with car accidents and serious falls, “night-eating syndromes” of bingeing on food, regurgitation in the oesophagus and peptic ulcer disease.

The surge in Pharmasset shares of nearly 85 percent to $134.17 in afternoon trade, created an opportunity for some options traders to reap a tidy windfall by taking profits off the table on call positions initiated this month. “As most investors probably don’t anticipate a higher bid to come in, some are using the surge in the share price to book massive profits,” said William Lefkowitz, options strategist at brokerage vFinance Investments in New York.

Several options participants said the well-timed trades in selected Pharmasset calls were not necessarily indicative of insider trading because they could reflect speculative bets.

JUST A COINCIDENCE?

But one trader said a call trade initiated just last week was too much of a “coincidence” at best and should draw the eyes of U.S. securities regulators. Traders often turn to calls hoping to profit on the rise of a stock’s price. Notable was the interest in Pharmasset option contracts, allowing investors to pay $80 apiece for the shares by December 16 expiration heading into the buyout news.

On November 16, data from Web site optionMonster showed the purchase of 358 December $80 Pharmasset calls for $1.55 per contract for a total premium of $55,490, said optionMonster co-founder Jon Najarian. On that day, the stock closed at $71.77. Early on Monday, those calls cost $55 per contract, making the position worth about $1.9 million, he said.

“This is extremely fortunate timing. Given that the value of these options moved up more than 3,300 percent, I think this trade may merit closer inspection by the regulators,” Najarian said.

A spokesman for the U.S. Securities and Exchange Commission, which looks into unusual stock and options trading, declined to comment. In addition, Pharmasset call volume was heavy on November 8 when 2,143 contracts changed hands, four times the average daily level, data from Trade Alert showed. Shares closed at $69.07 on that day.

The options on the anti-viral drug maker’s stock at that time averaged 565 calls and 582 puts per day. But the direction of those trades was not easily determined, said Trade Alert President Henry Schwartz. The volume consisted of a block of 495 December $80 calls that traded for $1.95 per contract and it was not clear if those calls were initiated by a buyer or a seller, he said.

The circumstances surrounding that call activity may have been tied to a presentation by Pharmasset at the Credit Suisse First Boston conference the next day. The announcement of a deal also fed a surge of options trading in Pharmasset as about 8,255 puts and 3,375 calls changed hands late on Monday, a combined volume that was 8.8 times its average daily level, Trade Alert data showed.

DaVita said it believed the inquiry was civil in nature and did not have details about the time period covered. The company said it has been notified that it will receive a request for documents, which may include a subpoena, from the Office of Inspector General for the U.S. Department of Health and Human Services.

The inquiry is being carried out by the U.S. Attorney’s office for the Eastern District of New York.

DaVita said that certain other providers that operate dialysis clinics in New York and provide services to New York Medicaid patients may also receive, or have received, a similar request for documents. The company said it intends to cooperate with the government and that no proceedings have been initiated against it at this time.

DaVita shares were down 5 cents at $68.75 in pre-market trade. They closed at $68.80 on Monday on the New York Stock Exchange.

In a study published in the British Medical Journal, the researchers added that the average lifespan of HIV positive patients should increase further with guidelines recommending they start treatment even earlier with modern, improved drugs. “These results are very reassuring news for current patients and will be used to counsel those recently found to be HIV-positive,” said Mark Gompels of Britain’s North Bristol NHS Trust, who co-led the study.

Around 34 million people globally have the human immunodeficiency virus (HIV) that causes AIDS, and the vast majority of them live in sub-Saharan Africa. Access to screening, diagnosis and early treatment with HIV drugs is limited in many poorer nations, but in wealthy countries like Britain their availability has made a big difference to many patients’ lives.

Gompels worked with Margaret May of Bristol University and used data from the UK Collaborative HIV Cohort study, which in 2001 began collating routine data on HIV positive people who had been attending some of Britain’s largest clinics since January 1996. They looked at patients aged 20 and over who started treatment with at least three HIV drugs between 1996 and 2008. Their analysis showed that life expectancy for an average 20-year-old infected with HIV increased from 30 years to almost 46 between the periods 1996 to 1999 and 2006 to 2008.

“We should expect further improvements for patients starting antiretroviral therapy now with improved modern drugs and new guidelines recommending earlier treatment,” May said in a statement about the work. The findings also showed that life expectancy for women treated for HIV in Britain is 10 years higher than for men. During the period 1996 to 2008, life expectancy was 40 years for male patients and 50 years for female patients, compared with 58 years for men and nearly 62 years for women in the general UK population.

In a comment on the findings, Elena Losina, a senior scientist at the Boston Brigham and Women’s Hospital in the United States said that although the progress in Britain was encouraging, it should also serve as “an urgent call” to increase awareness among patients and health workers about how effective HIV treatment can be — especially if started early.

“In turn this should increase rates of routine HIV screening, with timely linkage to care and uninterrupted treatment,” she said. “As these factors improve, the full benefits of treatment for all HIV infected people can be realized.”

But in a study of 410 Chinese adults with H1N1 flu (also known as swine flu), those who took the herb mixture typically saw their fevers resolve after 16 hours, versus 26 hours in patients in a “control group” whose only flu treatment was acetaminophen (Tylenol) if their fever passed 102 degrees F.

Tamiflu and another antiviral drug, Relenza (zanamivir), are usually reserved for people with severe cases, or those at high risk of flu complications like pneumonia.

The elderly and people with certain chronic health conditions, like heart or lung disease, are among those at high risk. It was necessary to first study the effects of maxingshigan-yinqiaosan in low-risk people with milder cases of the flu, said Dr. Lixing Lao, a professor at the University of Maryland School of Medicine in Baltimore who was not involved in the research.

But that’s also a limitation of the study, he noted. “In people with severe illness, this herb may not work. We don’t know yet,” said Lao, who also directs the traditional Chinese medicine research program at the university’s Center for Integrative Medicine.

And for healthy people with mild cases of the flu, there’s the question of availability. The herbs used in the study, which were heated and made into a beverage, have a long history of use in China for colds and flu.

They are also widely available in certain other countries, like Japan, Korea and Germany, according to Drs. Cheng Wang and Bin Cao of the Beijing Institute of Respiratory Medicine, who led the study.

It’s possible, the researchers say, that maxingshigan-yinqiaosan could offer a flu-fighting alternative in certain places where Tamiflu is scarce — like rural China.

But one of the key ingredients is ephedra, or ma huang, which is banned from use in dietary supplements in the U.S. and Canada. Those bans came after ephedra in weight-loss and body-building supplements was linked to heart attacks, strokes and deaths in some users.

That ban did not apply to ephedra’s use in Chinese medicine, where small doses are mixed with other traditional herbs — in contrast to the single, higher doses used in the banned supplements. However, Americans cannot go to their local store and pull maxingshigan-yinqiaosan off the shelf. They would need to go to a traditional Chinese medicine practitioner, Lao said.

Properly using the right mix of herbs, he said, “requires some knowledge.”

Lao said he was happy to see that a prestigious medical journal published a study on a traditional Chinese remedy as it is actually practiced — combining small doses of different herbs.

“If you only look at one herb, it may not work,” Lao told Reuters Health.

He also pointed out that the small herb doses used in practice limit the risk of side effects. In this study, two of the 103 patients who used maxingshigan-yinqiaosan alone developed nausea and vomiting. There were no side effects reported in the other three study groups.

The H1N1 swine flu broke out in 2009 and quickly spread around the world, killing more than 18,000 people, according to the World Health Organization (WHO). The WHO declared the H1N1 flu pandemic over in August of last year, but H1N1 has now taken over as the main seasonal flu strain in circulation around the world.

The U.S. Centers for Disease Control and Prevention (CDC) recommends Tamiflu and Relenza for treating the flu in people who are very sick or at high risk of complications.But in general, the agency says, flu sufferers who are otherwise healthy do not need the drugs. They can recover by getting plenty of rest and fluids, and using over-the-counter remedies like acetaminophen for fever.

According to Lao, the current findings are a “good first step” in showing through a controlled clinical trial that traditional Chinese herbs may also battle flu symptoms.

The CDC stresses, however, that the best defense against the flu is the flu vaccine. Experts recommend that everyone older than six months receive a yearly flu shot, which now includes a vaccine against H1N1.
